Cancer Prevention Care Coordinator: Pre-Health Gap Year Opportunity in Urban Community Medicine

Position Summary:
The Cancer Prevention Care Coordinator is the key advocate for all clinic programs that support patient cancer screenings, screenings that are in-house or via hospital partners. Reports directly to the RN Director of Care Coordination and collaborates closely with the Cancer Nurse Care Manager. Serves as key stakeholder for a variety of special programs including the Illinois Breast and Cervical Cancer Program (IBCCP), Illinois Medicaid Breast Cancer Navigation program, U-Screen study for Colorectal Cancer prevention in partnership with the University of Chicago. This position requires bilingual proficiency in Spanish and English.
The responsibilities include:
- Addressing urgent referral requests related to PCC patients with active cancer cases
- Supporting all patients with mammogram orders with scheduling, education and navigation barriers; includes all outreaching patients without insurance to enroll in free breast and cervical cancer screening program (IBCCP)
- Collaborating with area IBCCP agencies and PCC’s marketing team to promote broader awareness of cancer inequities in Black and Hispanic communities, and bring patients to high-quality care at PCC
- Communicating with area hospitals to obtain medical records required to maintain continuity and quality of care.
- Contacting patients overdue for any cancer screening care, including mammograms, colonoscopies, pap smears; engage to motivate clients on the importance of preventative cancer screenings, assist with scheduling and addressing any barriers, including transportation
- Assisting with data entry related to breast and cervical cancer case management and billing
This position requires the following experience/abilities:
- Ability to build client trust and provide excellent customer service during in-person and telephone contacts
- Ability to support a diverse patient population with unique needs
- Ability to independently problem solve and manage varied, time-sensitive responsibilities
- Ability to effectively communicate and work with a team
- Technical skills to navigate numerous insurance and health system portals to support patient care
- Ability to speak Spanish preferred but not required
- Personal transportation is helpful but not required
- Experience with MS Office applications Excel and Word
All Gap Year Program Participants will learn the following on the job:
- Extensive training in EHR (AthenaOne), including patient chart creation, appointment scheduling, documentation of care provided, interpreting and responding to clinical communication in charts from outside parties
- Extensive experience with understanding and helping patients address health-related social needs (finding appropriate resources, educating patients in navigating these systems)
- Extensive understanding of the healthcare system that causes inequitable outcomes for underserved patients, including patients of minoritized and low-income backgrounds; nuanced understanding of ways to help patients overcome barriers to quality care
Other on-the-job experiences in the Cohort include
- Quarterly cohort lunches with PCC’s Chief Medical Officer
- Physician/provider 1:1 mentorship (1 hr/month); opportunity for off-the-clock shadowing as desired
- Professional development programming led by Nurse Director of Care Coordination and managers, including Tips for Applying and Interviewing for Healthcare Postgrad programs (Medical school, etc.); Overview of Health Insurance system and Illinois Medicaid; Tips for Building Trust in a Work and Clinical setting
Potential candidates should consider the following details before applying
- This is a full-time (40 hours a week) at-will employment opportunity
- Each position has a home clinic, is a combination of virtual and in-person work, and entails predominantly telephonic patient support, with varying volumes of face-to-face care.
- It is strongly preferred that applicants plan for at least a 13-month commitment, so they may train their replacement at the end of the academic year.
